Candidate Compliance

Proof of Address, Training & Immunity Evidence What You Need

Everything we need from you to get your compliance file complete, so you can start booking shifts without delay. Work through the three sections below, then send your documents to your SHR compliance officer.

1
Step One

Proof of Address

Please provide two proofs of address.

Both must show your full name and current home address, and each must come from a different source (for example, one bank statement and one utility bill, not two statements from the same bank).

Each document must be dated within the validity period shown below.

Document Type Must Be Dated Within
Bank Statement 3 months
Credit Card Statement 3 months
Bank or Building Society Account Opening Confirmation Letter 3 months
Utility Bill (Gas, Electricity or Water) 3 months
Phone Bill (Landline Only) 3 months
Document from a Central Government Agency (e.g. DWP or HMRC) 3 months
Mortgage Statement 12 months
Council Tax Statement or Bill 12 months
P45 / P60 12 months
UK Photo Driving Licence Valid until expiry

Helpful Tips

  • Mobile phone bills are not accepted, but landline bills are.
  • Online statements and e-bills are fine, as long as they are clear, complete and show your name, address and date.
  • Please send whole documents rather than cropped screenshots.
  • If you have recently moved, let your consultant know and we’ll help you find suitable documents.
2
Step Two

Mandatory & Statutory Training

Please provide completion evidence for each of the modules below.

Certificates must show your name, the module title, the completion date and the training provider. Each certificate must still be within its validity period when you start a placement.

Module Valid For
Health, Safety and Welfare 3 years
Fire Safety 2 years
Moving and Handling 2 years
Information Governance and Data Security 1 year
Equality, Diversity and Human Rights 3 years
Safeguarding Adults 3 years
Safeguarding Children 3 years
Preventing Radicalisation 3 years
Infection Prevention and Control 1 year
Resuscitation (Basic Life Support) 1 year
Conflict Resolution 3 years

Helpful Tips

  • These modules follow the Core Skills Training Framework used across the NHS, so certificates from your NHS employer, e-Learning for Healthcare or another accredited provider are usually accepted.
  • Some modules expire after only 1 year, so check your Information Governance, Infection Prevention and Basic Life Support dates first.
  • We’ll remind you before anything expires, so your compliance stays current and your shifts continue without interruption.

Step Three

Immunity Evidence

Please provide evidence of immunity for each requirement that applies to your role.

Occupational health checks protect you, your colleagues and your patients from infections that can pass between people in clinical settings.

Is your role EPP or non-EPP?

What is an EPP?

Exposure-prone procedures (EPPs) are procedures where an injury to you could result in your blood coming into contact with a patient’s open tissues, such as some types of minor surgery.

Which applies to me?

Most primary care roles are non-EPP. Everyone needs to provide the non-EPP evidence. If your role involves EPPs, you’ll also need the additional blood tests listed below.

Your consultant or compliance officer will confirm which applies to you.

What we need from you

Applies To Requirement Evidence Required
All roles (Non-EPP) Hepatitis B Antibody A blood test report showing your antibody (titre) level
All roles (Non-EPP) Measles, Mumps and Rubella (MMR) A blood test showing immunity, or records of two MMR vaccinations
All roles (Non-EPP) Varicella (Chickenpox) Confirmation that you’ve had chickenpox, or a blood test showing immunity
All roles (Non-EPP) Tuberculosis (TB) A BCG scar verified by a GP or occupational health nurse, or a Mantoux test
EPP roles (additional) Hepatitis B Surface Antigen Blood test result from an identity-validated sample
EPP roles (additional) Hepatitis C Blood test result from an identity-validated sample
EPP roles (additional) HIV Blood test result from an identity-validated sample

What is an identity-validated sample?

For EPP clearance, your blood sample must be taken by an occupational health service that checks your photo ID at the moment the sample is taken. This confirms the result genuinely belongs to you. Results from samples that were not identity-validated cannot be accepted for EPP clearance.

Understanding your Hepatitis B and EPP results

Use this table to see what your results mean and whether any further action is needed.

Result What It Means
Hepatitis B surface antigen (HBsAg)
Negative No restriction
Positive Restricted from performing EPPs
Hepatitis B surface antibody (anti-HBs)
Above 100 mIU/mL Immunity confirmed
No further blood tests needed.
10 to 100 mIU/mL Booster needed
A booster dose of Hepatitis B vaccine is needed. If a repeat test then shows above 100 mIU/mL, no further blood tests are needed. If not, you’ll be treated as a non-responder.
Below 10 mIU/mL Non-responder
An annual HBsAg blood test is required.
Below 10 mIU/mL with positive Hepatitis B core antibody (HBcAb) Considered naturally immune
No further vaccination, blood tests or recalls are needed, but you must tell us if you start immunosuppressive treatment.
Hepatitis B viral load
200 IU/mL or above Cannot be cleared for EPP
HIV viral load
Below 200 copies/mL on two tests taken 12 weeks apart, while on combination antiretroviral therapy (cART) Required for EPP clearance

When you’ll need a blood test for MMR, varicella or TB

Requirement When It’s Needed Result Needed
MMR antibody (measles, mumps, rubella) If you don’t have records of two MMR vaccine doses A positive antibody result is accepted in place of vaccination records
Varicella (chickenpox) If you have no history, or an uncertain history, of chickenpox or shingles. If you were born or raised in a tropical or subtropical climate, you’ll be screened regardless of your history. A positive result means you’re immune. If negative, you’ll be offered the vaccine. A history of chickenpox can be self-declared.
TB (Mantoux or IGRA) If your role involves contact with patients or clinical material A negative Mantoux or IGRA test within the past 5 years, or a documented BCG scar or vaccination. You must not have visited a country with a high incidence of TB in the 12 weeks since your test.

Helpful Tips

  • Send the laboratory report or occupational health letter, showing your name, the test, the result and the date.
  • If you’re missing evidence or need a booster, tell your consultant early. Vaccination courses and repeat tests can take several weeks.
